The blade tip carries a sharp cutting edge oriented to access the mesial root surface from within the sulcular space without contacting adjacent bone. The shank transitions through a defined curve, positioning the blade at an angle that clears the adjacent tooth crown during insertion. The elongated, tapered handle — visible in the single-ended instrument profile — supports a palm-and-thumb grip for controlled apical pressure delivery without torque on the surrounding alveolar bone.
Unlike Periotomes, which primarily compress the PDL space through thin wedging blades worked circumferentially, Desmotomes function by incising the soft tissue attachment and PDL directly — severing rather than displacing the ligament.
